We are hiring part-time and per diem Registered Nurse (RN) Case Managers to join our dynamic care team at Hospice of Western & Central Massachusetts in Worcester County!

This is an on-the-road position that requires the candidate to have insured and reliable transportation. Weekly pay. 8a-4:30p. $48 to $50 an hour.

The RN Case Manager plans, organizes, and directs hospice care to our patients across the region and directs services to meet the needs of the patients and their families/caregivers.

Education & Experience:

Must hold an active and unencumbered RN license in the state.
Hospice, home care, or acute care experience preferred.

Duties & Responsibilities:

Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
Assess and evaluate patient status.
Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
Communicate with the physician regarding patient needs and report changes in conditions.
Supervise ancillary personnel.
Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions.
Work in cooperation with the family/caregiver and hospice interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
Ensure equipment and other necessary services are available.

Specific Requirements:

Must be willing to travel.
Must be able to read, write, speak, and understand the English language.
Must be a licensed driver with an insured automobile in working order.

Job Responsibilities Providing resident-focused, high-quality care while working as a RN in post-acute facilities Developing individualized care plans for residents in coordination with the wider medical team Reviewing care options with clinicians, nursing staff, residents, and their family members Setting up and using medical equipment and devices as needed for effective care Administering medications and providing treatments to residents consistent with their care plan Measuring and monitoring resident vital signs and drawing fluids as needed for labs and testing Ensuring accurate documentation of relevant information such as interventions and responses, vital signs, and resident medical histories Training and supervising nursing professionals and support staff as needed, consistent with scope of practice limitations Supporting staff with incident investigations and reporting Being accountable to follow and adhere to resident care plans Delivering care consistent with all applicable laws, policies, and standards of care Qualifications and Skills Current Massachusetts state license to practice as a Registered Nurse (RN) Successful completion of an approved nursing program Minimum of 6 months of experience ( RN or related role) Advanced skills in multitasking, organization, and communication Ability to lift a minimum of 50 lbs Find Real Work-Life Balance With IntelyCare If you're ready to finally have a career and a full life to go with it, then your search for Registered Nurse (RN) jobs brought you to the right place.
