As a Nursing Admission Specialist, you will initiate and/or resume home care for patients of all ages and health backgrounds in lower Cuyahoga County and upper Summit County, offering comprehensive assessments, care plans and ongoing support to meet their unique needs. Your efforts help them stay independent and confident when managing their own care, ultimately reducing hospital readmissions and poor outcomes. This position offers rewarding experiences and multiple resources for enhancing your nursing skills, education and professional development. 

A caregiver in this position works days from 8:00AM – 5:00PM, with one on-call shift per week. The primary on-call shift is nights from 5:00PM – 8:00AM and the secondary on-call shift is 5:00PM – 9:00PM.

A caregiver who excels in this role will: 

  • Conduct and submita complete and accurate Comprehensive Assessment in accordance with OASIS reporting requirements and agency policies and procedures.


  • Identify immediate care and support needs and obtain orders to address psychosocial and treatment priorities.


  • Make appropriate admission decisions based on patient eligibility, acuity and safety.


  • Obtain consent for care.


  • Demonstrate knowledge of coverage criteria based on payer source, including:skilled needs, intermittent care, homebound status, and physician ordered care.


Minimum qualifications for the ideal future caregiver include: 

  • Graduate of an accredited school of Professional Nursing


  • Knowledge of Home Care and the OASIS data set


  • Experience with Infusion therapy, venipuncture, diabetes management, wound/ostomy care, urinary catheterization, enteral feeding, Oxygen Therapy, administration of injectable medication and Left Ventricular Assistive Devices


  • Current state licensure as a Registered Nurse (RN)


  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)


  • Valid Driver's License (VDL) upon hire and per policy 8-175 thereafter


  • Proof of automobile $100,000/$3,000,000 insurance coverage and per policy 8-175 thereafter


  • One year of current clinical experience as an RN


  • One year of experience in an acute medical/surgical or home care setting


Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN)


  • Six months of home health care experience

Physical Requirements:

  • Must be ambulatory and able to drive to patients homes and potential customers, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
