Overview

The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to the assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate the patient?s plan of care and maximize the ability to remain in a community setting. In collaboration with the physician, the nurse identifies at risk patient populations who require care management services. The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary health care team, as well as community resources. The Ambulatory Care Nurse understands the needs of the organization and supports the mission, values, and management of patient care services. The ACN actively supports and incorporates ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.

Participate on interdepartmental committees to foster teamwork and improve the delivery of quality patient care. 13. Participate with Referral sources to identify patient population needs, identify referral process improvements and staff educational opportunities. 14. Other duties as assigned. Preferred & Required Qualifications Education BSN is required. MSN is preferred Licensure & Certifications Current RN licensure in the state of NC or compact license from participating state is required. BLS (may be renewed during orientation) Valid NC Driver’s license Professional Work Experience 5 years of clinical practice with at least 2 of those years being in supervisory role required. Experience in home-based care environments or care coordination required. Working knowledge of Medicare and Medicaid regulations including JCAHO Computer skills including Word and Excel preferred. Excellent communication & organizational skills required.
