Position Summary

We are seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our hospice care team serving the Lehigh Valley area. This role provides hands-on, comfort-focused care to hospice patients in their homes and other care settings, supporting dignity, comfort, and quality of life at the end of life.

Key Responsibilities

Provide direct patient care including bathing, grooming, toileting, and assistance with activities of daily living (ADLs)
Measure and document vital signs as appropriate
Assist with mobility, transfers, and positioning to promote comfort and safety
Observe and report changes in patient condition to the hospice nurse
Offer emotional support and compassionate presence to patients and families
Maintain a clean and safe patient environment
Follow individualized plans of care established by the interdisciplinary team
Accurately document care provided in accordance with hospice policies and regulations
Maintain patient dignity, privacy, and comfort at all times

Qualifications

Current Certified Nursing Assistant (CNA) certification in Pennsylvania (required)
Previous hospice, home health, or long-term care experience preferred
Compassionate, patient-centered approach to care
Strong communication and time-management skills
Ability to work independently in the field
Valid driver’s license with reliable transportation
