Main Purpose of Job

This position provides care coordination for Maternal Child Health clients to promote healthy behaviors and access to care by appropriate nursing assessment, planning, linkage, monitoring, and evaluation; responsible for the nursing services in the Lead and Asthma program; educates and assists clients in understanding present health status, developing self-care measures, and improving and maintaining maximum health care and safety.

Education: A Bachelor’s degree in nursing from an accredited college or university.
