Job Overview

The LVN Supervisor, Utilization Management - Clinical position is responsible for overseeing and managing the day-to-day operations of the Utilization Management Department, health plan delegated activities, and medical management initiatives. This position works closely with the Manager, Director, Medical Director, and VP to evaluate and monitor medical appropriateness determinations and care coordination activities. This position is responsible for developing and implementing strategic plans that will directly impact cost containment and improve patient outcomes.

Minimum Requirements

Graduated from an accredited LVN school with a current LVN license issued by the State of California, Vocational Nursing. RN, preferred
AA degree or equivalent work experience preferred
A minimum of 3 years of experience in a managed care environment is required.
Minimum of 1 year of leadership experience preferred (lead or supervisor)
Knowledge of State and Federal regulations is required.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IHH currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 11:00pm to 7:30am, 5 days on, 2 days off. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health. Arrange transportation for clients to and from medical appointments, as needed Report emergency situations to Supervisor/Manager and document incident Seek proper level of care to assess and treat medical complication Initiate medical response system when appropriate Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility. Administrative/Record Keeping: Enter patient information and case notes into EHR and other documentation systems. Complete and update medication, vitals, progress notes and care plan Perform administrative tasks such as answering phones and filing paperwork. Prepare patient reports for billing. Maintain proper inventory of medical supplies and medications. Compliance/Safety: Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards.
