Key Responsibilities

  • Administer skilled nursing care under RN supervision.
  • Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.
  • Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.
  • Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required  documentation.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
  • Understand and adhere to the agency’s process for managing and addressing customer complaints.
  • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
  • Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.   

Qualifications

  • Must have a current LPN or LVN license (state-specific).
  • Graduate of an accredited nursing program.
  • Valid BLS certification.
  • Minimum one year of nursing experience (home health or medical/surgical preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
