Franciscan Physician Network is seeking a full-time Nephrology Advanced Practice Provider to join our team at Franciscan Health Hypertension & Nephrology located in Lafayette, Indiana. Nurse Practitioner or Physician Assistant will support the Nephrology department by seeing patients in the clinic and in the outpatient dialysis units. There are 4 dialysis units and 4 clinic locations. The APP will rotate one to two days per week at these sites with one physician and one APP.

Ideal candidate will have past or present nephrology experience, strong focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.


WHAT YOU CAN EXPECT:

  • Traditional full-time schedule
  • Clinic hours: Monday – Friday, 8:00 a.m. – 5:00 p.m.

IBMT is a National Marrow Donor Program (NMDP) collection center for Bone Marrow & Peripheral Blood Stem Cells. There is a Stem Cell processing laboratory on site. The program has both inpatient and outpatient services. Immune Effector Cell Therapies and Clinical Research are also part of the program. WHAT WE NEED: Bone Marrow Transplant, Hematology and/or Oncology experience preferred, but not required. Ideal candidate would thrive in a fast-paced environment and be team oriented. Job Description Under the general supervision of the Preoperative Anesthesia Clinic Medical Director, Pain Clinic Medical Director, and Chief of Anesthesia, and in accordance with standardized procedures, the Nurse Practitioner (NP) role will work clinically in the Preoperative Anesthesia Clinic (PAC). The Preoperative Anesthesia Clinic is an ambulatory clinic responsible for the preoperative assessment and optimization of patients undergoing surgery. The clinic offers telemedicine and in-person consultations. The NP role provides clinical and operational support to the Preoperative Anesthesia Clinic at Zuckerberg San Francisco General Hospital. General responsibilities are obtaining a health history, performing a physical assessment, identifying problems and formulating a plan that includes further diagnostic evaluation, risk stratification and counseling. Qualifications Required Qualifications: Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master’s degree in nursing or a Doctor of Nursing Practice (DNP). Nurse Practitioner 1 (less than six (6) months of experience as an NP): At least one year of recent work in outpatient neurology and/or neurosurgery clinic Nurse Practitioner 2 (at least six (6) months of NP experience): At least two years of recent work in outpatient internal medicine or critical care. A minimum one (1) year of recent clinical experience as a Nurse Practitioner.
