Lead & Elevate New Hire Success

  • Guide clinicians through hands-on field orientation, ensuring confidence in:

    • Start of Care (SOC), ROC, Recertifications, and routine visits

  • Partner closely with preceptors and leadership to ensure a consistent, high-quality onboarding experience

  • Evaluate readiness, provide feedback, and support clinicians as they transition to independence

Drive OASIS & Documentation Excellence

What We’re Looking For

We’re seeking a true home health expert who thrives in a mentorship role:

  • Registered Nurse (RN) with active, unrestricted license (multi-state/compact preferred)

  • 3+ years of Skilled Home Health experience strongly preferred (not suitable for candidates without strong SHH background)

  • Deep expertise in:

    • OASIS accuracy and documentation

    • Medicare regulations and home health compliance

    • End-to-end home health workflows (SOC through discharge)

  • Proven experience as a preceptor, mentor, or informal clinical leader

  • Strong ability to teach, coach, and influence clinicians in the field

  • Comfortable working across multiple locations in a fast-paced, evolving environment

  • Willingness to travel 50% within the region

The base salary for this position is $90,000 to $95,000 annually, based on the company’s good faith estimate at the time of posting
