What You'll Do as a Hospice RN / RN Case Manager:

  • Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.

  • Deliver and document skilled, hands-on nursing care based on each patient’s individualized Plan of Care—in their home, assisted living, or wherever they call home.

  • Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.

  • Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans.

  • Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance.

  • Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance.

  • Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions.

  • Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards.

  • Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient.

  • Help patients and families understand and navigate topics such as: 

  • Medication administration 

  • Hospice philosophy and services 

  • Symptom and pain management 

  • End-of-life processes and expectations

About You

Qualifications – What You’ll Bring:

  • Active RN license in the state of employment (or eligible to obtain).

  • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.

  • A genuine hospice heart: compassionate, empathetic, and patient-centered.

  • Comfortable providing care in diverse settings, including private homes and facilities.

  • Strong communication and critical thinking skills in emotionally sensitive situations.

  • Valid driver’s license, auto insurance, and reliable transportation.

  • CPR certification required.

Preferred Experience (Not Required):

  • Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)

  • Admissions RN, wound care, float pool, travel nursing

  • Experience with terminally ill patients or serious illness support

RN Hospice - On Call - Nights

Accomac, Virginia Overview Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The RN may work in a variety of settings (typically in a home care type setting) and roles within Riverside Health System. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. The RN On-Call provides support to the agency after hours via phone or patient visit as necessary. Will take calls from patients regarding issues or concerns and triage the clients needs as to whether a visit is necessary or if the problem can be handled via patient/caregiver education via phone. Will be responsible for taking any after hours referrals and forwarding information to the Care Manager on call. Is responsible for assessing all of the patient's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or via patient visit. The On-Call nurse works effectively within RHCD to maintain an environment of excellence which is patient focused and provides timely, appropriate, compassionate, quality client care. Supports the Hospice Philosophy. What you will do Assesses, plans, implements, and evaluates the total nursing care of assigned patients on an intermittent visiting basis. Provides support to the agency after hours via phone or patient visit as necessary. Takes after hours referrals and forwards information to the Care Manager on call. Manages as assigned caseload. Takes calls from clients regarding issues or concerns and triages the clients needs as to whether a visit is necessary of the problem can be handled via patient/caregiver education via phone. Accurately completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Required documentation is to be completed no later than the last day of the month in order to provide timely billing. Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Assesses client's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or patient visit. Demonstrates assessment ability by applying standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops an initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points. Supervises RNs and LPNs in providing services. Participates in training and support of all team members, volunteers and students as needed. Participates in multi-disciplinary team meetings and discussions regarding quality and outcomes. Delegates and prioritizes tasks appropriately to facilitate quality patient care and ensure patient care needs are met. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems to meet need. .
