Kidney Transplant Nurse Practitioner / Physician Assistant
Location: East Jefferson General Hospital - Metairie, LA

Day shift - Inpatient / Outpatient
Position Summary
The Kidney Transplant Nurse Practitioner / Physician Assistant provides comprehensive medical care and follow‑up for abdominal transplant patients (Liver, Kidney, Pancreas) across the continuum of care. Working collaboratively with transplant nephrologists and the multidisciplinary transplant team, this role focuses on health assessment, treatment planning, preventive care, and ongoing evaluation of patient outcomes.
The APP will provide essential outpatient coverage to support transplant nephrologists in managing increasingly complex, high‑risk patients and growing referral volumes driven by revised intake protocols.
Key Responsibilities
- Provide direct outpatient care for kidney transplant patients, including pre‑ and post‑transplant management
- Perform comprehensive physical examinations and preventive health assessments in accordance with physician‑directed protocols
- De
- velop and implement individualized treatment strategies to meet identified patient needs
- Evaluate patient responses to medical and nursing interventions and adjust plans of care accordingly
- Provide outpatient coverage that enables transplant nephrologists to focus on high‑risk patients and increased new referral volumes
- Manage wait list patients and conduct comprehensive post‑transplant monitoring, including immunosuppression management
- Monitor transplant‑related complications,
- lab results, and long‑term graft function
- Collaborate closely with transplant nephrologists, coordinators, pharmacists, nurses, and other interdisciplinary team members
- Educate patients and families on transplant care, medications, and long‑term health management
- Maintain accurate, timely medical documentation in the electronic health record
- Support quality, safety, and patient‑experience initiatives within the transplant program
- Assist with Waitlist management of kidney transplant patients and management of post kidney transplant patients.
Qualifications
- Master’s degree or higher from an accredited Nurse Practitioner or Physician Assistant program
- Active Nurse Practitioner or Physician Assistant license in the state of Louisiana (or eligibility for licensure)
- National certification as an NP or PA
- Experience in abdominal/kidney transplant, nephrology, internal medicine, or acute care strongly preferred as RN or APP.
- Critical Care experience preferred.
- Strong clinical judgment and ability to manage complex transplant patients
- Excellent communication, collaboration, and organizational skills
- Basic Life Support (BLS) from American Heart Association
