A Brief Overview
Provides comprehensive cardiovascular education following an established education care plan. Will document the work he/she completes and any pertinent observations as a result of any interventions performed. Will communicate further cardiac patient needs to the other members of the care team, including but not limited to: nurse, physicians, APRNs, social services, nurse managers, subsequent Care Manager as needed. Other duties include, but not limited to: • Performs daily review of EPIC daily reports. Follows appropriate cardiac protocols as ordered by the physician/PA/APRN • Formulates a prioritized and goal directed educational plan of care, which is based on patient's presenting problems, learning needs, and projected outcomes related to cardiovascular disease. Utilized established teaching materials for all education provided. • Will offer and schedule appointment for follow up with the appropriate provider within the appropriate timeframe per the established practice guidelines. • Maintains inventory of cardiac education materials and communicates to appropriate personnel the need for equipment (i.e., scales, MCOTS, etc.) • Will identify psychosocial barriers to achieving cardiac self-care management skills and report to appropriated primary or ancillary services for assistance (i.e., cost, transportation, home health, etc.) • Participates actively in clinical education activities, including community-based learning activities. Participates in system wide/department patient care quality improvement activities as well as standards development. Participates actively in Heart Failure Steering committee and departmental meetings. • Perform pulmonary artery pressure and heart rate reading using the CardioMEMS Hospital Electronics System for inpatients with CardioMEMS device as needed. • Perform placement and education of Mobile Cardiac Outpatient Telemetry (MCOT) device for patients being discharged from inpatient status as directed by Neuro Hospitalist and Cardiology providers. Provide necessary communication to Heart Center Device Clinic Medical Assistant for enrollment.
Education Qualifications
Bachelor's of Science in Nursing (BSN) Preferred
Experience Qualifications
Cardiology Experience in the clinic or hospital setting. Preferred

Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. (Required proficiency)
Willingness to learn necessary computer skills and the ability to document in the electronic health record. (Required proficiency)
Licenses and Certifications
Registered Nurse - KSBN A multistate license with the ability to practice within the State of Kansas is also accepted. Required
Basic Life Support - BLS RQI, CPR or ACLS certifications may be required in lieu of or in addition to BLS depending on department. Must have within 90 days of hire.
