Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. Provides oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities. Ensures the timely start of patient care within recommended regulatory guidelines. Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate). Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness. Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician. Conducts regular OASIS reviews for all patients. Meets with all direct care team members on a regular basis providing guidance, coaching and communicating information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff. Communicates frequently with the Branch Director or Administrator on key performance metrics. Remains available for field work, as needed. What is required? Associate degree in a Nursing related field, Bachelor preferred 2 years of home care experience in Home Health or Hospice environment 1 year of supervisory experience required Current, unrestricted RN license valid for the state of work Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office Able and willing to travel up to 50% as needed to support business and patient needs. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Administration or Branch Director. Maintains communication with the Branch Director or Clinical Team Manager, RN Case Manager, and Scheduler, with updates on changes in schedules, patient data or needs, physician orders or special instructions. Participates in continual review and evaluation of clinical practices and learning activities to update home health nursing skills. Performs other duties/projects as assigned. What is Required? Graduate of an accredited school of nursing. 1+ year of experience as a nurse in a clinical setting. Registered Nurse (RN) in the state or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC). Must be able to travel to patient and client homes with dependable vehicle, drivers license and maintain insurance as required by state law.
