Position Details: Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS! What you will do: Plan and prioritize visits based on client needs and physician orders. Assess clients at admission and key intervals and document accurately. Coordinate care and referrals with other disciplines as needed. Educate clients and caregivers on therapy programs and home exercises. Monitor progress and report changes promptly to the physician. Ensure compliance with laws, regulations, and agency policies. Complete documentation at point of care in a timely manner.
