Facilitate referrals and care transitions to specialty behavioral health, substance use treatment, social services, or community resources as clinically indicated, and develop relapse prevention and self-management plans for patients achieving treatment goals or transitioning from the caseload. Required Education and Experience Master’s degree or higher in Social Work, Counseling, or Psychology from an accredited institution. Minimum 1–3 years of clinical experience providing behavioral health assessment and treatment. Required License/Certifications Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Psychological Associate (as permitted by Texas law)

Consistently follows hospital policy regarding verification of patient identification prior to obtaining specimen and ensures specimens are appropriately labeled with patient identifiers, date and time of collection and initials. Ensures lab charges and patient charges are completed and accurate on a daily basis either in the computer or on paper as indicated in the designated areas.
