In collaboration with the patient/family, physicians, and the interdisciplinary team, the Case Manager facilitates the patient's progress through the acute episode of care in an efficient and cost effective manner. Monitors the level of care, initiating evidence based protocols and order sets according to the patients' admission diagnosis, influencing the progression of care, facilitating the patients' navigation through the acute care episode, and proposes an appropriate transition plan within 48 hours of admission. The Case Manager serves as an advocate for the patient and family throughout the entire acute episode of care. As a nurse practitioner employed by FCCMG, you are expected to contribute to the clinical mission, as well as the research (clinical) and academic agenda of the Department of Medicine and the Center as a whole. You will play an essential role in supporting clinical programs to satisfy these goals. Predominately, the nurse practitioner's time will be devoted to clinical responsibilities. Depending on your area and as volume dictates, you may be responsible for covering various aspects of patient care, including but not limited to, outpatient clinic sessions, inpatient consultation liaison services, medication and therapy management and weekday and weekend call rotation at the discretion of the Section Chief and the Chairman of Medicine. Your clinical schedule will be coordinated in accordance with departmental needs and consideration of your practice goals.
