Responsibilities: Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician. Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing. Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs. Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team. Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols. Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions. Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care. Qualifications: Licensure: Valid LPN/LVN license in Nebraska or Multi-state Education: Graduation from an accredited LPN/LVN program. Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred. Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations. Communications: Excellent verbal and written communication skills.
