The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.


Job Requirements

  • Bachelor Degree Nursing Required
  • Registered Nurse State of Employment Required
  • Three years of experience required, with one year in specialty.
  • Strong psychosocial assessment and clinical skills
  • Basic Life Support American Heart Association Required

  • American Nurse Credentialing Center preferred

Physical Requirements - Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally.
