Medical Coordinator (LVN) - Swing Shift

$34 - $36 / hour

Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The Medical Coordinator is responsible for delivering high quality medical care and influencing positive
outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights.

The pay range for this position is $34.00 - $36.00 per hour, depending on experienceThis position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential.

The schedule for this position is 3:00pm to 11:30pm. Workdays may vary.

Responsibilities

Client Care:

  • Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes
    • Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals
    • Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list
  • Establish and maintain a care plan
    • Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs.
  • Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
  • Link clients to medical services when necessary (insurance, PCP, specialists, referrals).
  • Coordinate home health visits (if needed) and follow-up on medical appointments.
  • Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health.
  • Arrange transportation for clients to and from medical appointments, as needed
  • Report emergency situations to Supervisor/Manager and document incident
    • Seek proper level of care to assess and treat medical complication
    • Initiate medical response system when appropriate
  • Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility.

Administrative/Record Keeping:

  • Enter patient information and case notes into EHR and other documentation systems.
  • Complete and update medication, vitals, progress notes and care plan
  • Perform administrative tasks such as answering phones and filing paperwork.
  • Prepare patient reports for billing.
  • Maintain proper inventory of medical supplies and medications.

Compliance/Safety:

  • Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies and DHS standards.
  • Provide patients with proper compliance forms upon admission
  • Utilize infection control measures according to OSHA standards.
  • Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste.
  • Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room.

Qualifications

Required:

  • Current and non-restricted LVN license in California
  • 2 years of relevant experience.
  • High School Diploma or equivalent.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Valid CA Driver’s License.

Preferred:

  • Availability to work weekends and nights
  • Home Health nursing experience and experience working with populations dealing with addiction and behavioral health conditions.
  • Bilingual in English and Spanish.
  • Proficiency in Microsoft Office (Word, Excel, Outlook, etc.) and KIPU or other electronic health record systems.
