In this role you will be working in a next generation virtual heart clinic that dramatically expands access to care for America's most vulnerable seniors. The clinic focuses on providing highly accessible, high quality care for members with general cardiology conditions. As part of the clinic you will also provide dedicated clinical services during part of your clinical time to our heart failure clinic which is a highly tailored, specialty center focused on providing best in class heart failure management including optimizing guideline directed medical therapy, providing health coaching, and managing volume overload. You will be a key member of our interprofessional team. On a day-to-day basis you will work closely with heart failure RNs who are assigned as clinical guides for each patient in the heart failure clinic. You will also work closely with an interdisciplinary team including physicians, pharmacists, social workers and medical assistants. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits. Responsibilities will include: Conduct focused and thorough assessments of patients with general cardiology and heart failure needs through virtual consultations including ordering diagnostics as needed, interpreting labs and imaging data, and developing a treatment plan in collaboration with the specialty care clinic team. We expect that 80-90% of your time will be performing virtual visits. Initiate, titrate, and manage guideline-directed medical therapies (GDMT) for a diverse group of members with heart failure. Provide proactive management of heart failure and comorbid conditions, and address potential barriers to optimal care to mitigate the risks of heart failure exacerbation and hospitalizations. Assess and intervene in weight fluctuations and acute volume overload management of patients who are enrolled in the clinic. Assess, diagnose, and manage the range of general cardiology conditions that are suitable for virtual care including stable coronary artery disease, hypertension, hyperlipidemia, stable atrial arrhythmias, peripheral arterial disease and chronic venous disease. You will be ordering in-home diagnostics including echocardiograms, EKGs, and Holters and working with community cardiology practices for other diagnostics including stress testing and advanced cardiac imaging. Collaborate closely with other members of the care team including PCPs, cardiologists and other Devoted team members including pharmacy, clinical nursing, social work, as well as interfacing with family members and caregivers to coordinate holistic care for the member, to ensure continuity of care and deliver a collaborative care plan. Initiate, titrate, and manage guideline-directed medical therapies (GDMT) for a diverse group of members with heart failure. Provide proactive management of heart failure and comorbid conditions, and address potential barriers to optimal care to mitigate the risks of heart failure exacerbation and hospitalizations. Assess and intervene in weight fluctuations and acute volume overload management of patients who are enrolled in the clinic. Assess, diagnose, and manage the range of general cardiology conditions that are suitable for virtual care including stable coronary artery disease, hypertension, hyperlipidemia, stable atrial arrhythmias, peripheral arterial disease and chronic venous disease. You will be ordering in-home diagnostics including echocardiograms, EKGs, and Holters and working with community cardiology practices for other diagnostics including stress testing and advanced cardiac imaging. Under the clinical direction of the Director, Clinical Practice and the administrative direction of the Clinical/Practice Administrator, the Operations Coordinator participates in the planning, leading, and organizing of activities relating to quality patient care in the practice. Direct clinical patient care would be provided less than twenty percent of the time in this role. The Operations Coordinator may oversee multiple sites or departments within RMG. Responsibilities: Supervise and direct the office’s clinical support staff, manage workload distribution and prioritization of tasks. Participate in interviewing, hiring, training, progressive discipline, dismissals, and performance appraisals. Oversee and direct day to day office clinical operations within budget guidelines. Assist with managing providers schedule templates and optimize patient care hours to enhance productivity. Schedule staff and allocate resources across hours of operation to maximize patient access and satisfaction, provider productivity and efficiency, and staff productivity. Responsible for front-line communication with staff to receive and provide feedback regarding clinical workflow, quality and productivity. Ensure that service excellence is delivered and perform service recovery whenever necessary. Manage medical surgical supplies within budget. Ensure accuracy of all data collection and data entry by site staff through training and compliance monitoring and facilitate and oversee flow of information to and from central administrative offices. Conduct office staff meetings on a regular basis in coordination with practice leadership. Attend and actively participate in leadership meetings. Ensure that patient dignity and confidentiality are always maintained. Additional Functions Review and approve automated time reports for accuracy. Develop tools to support staff and enhance efficiency. Provide input into staff performance reviews. May participate with manager in conducting staff performance reviews. Provide one-to-one coaching and mentoring as needed. Monitor daily transaction activity to ensure compliance with procedures. Resolve complex or critical situations involving patients, staff and customers: consult with manager for guidance as needed (e.g., complaints, discipline, etc.). Position Summary The DSCC Home Care nursing consultant provides care coordination services to families eligible for DSCC's Home Care program. The Home Care program serves Medicaid non-waiver participants under 21 years and those eligible for the Persons who are Medically Fragile, Technology Dependent (MFTD) waiver program. This position is responsible for engaging and building strong partnerships with the families enrolled in the program through monthly interactions, completion of comprehensive assessments, person-centered care plans, and engagement with multiple stakeholders. It also offers consultation to other members of the multi-disciplinary team utilizing skills and knowledge acquired from academic training and professional experience as a Registered Nurse. Duties & Responsibilities  Under the direction of the regional manager, performs active care coordination services by completing comprehensive health assessments, identifying families' strengths, and developing a person-centered service and care plan.  Ensure that the participant and/or legally responsible adult understand the waiver services furnished to the participant, estimated frequency, and provider type.  Facilitates 30-day ( or as needed) monitoring of the person-centered care plan, assesses/determines status change, prioritizing unmet needs and location of resources.  Utilize a culturally – competent approach as guided by the university to support families’ cultural values and traditions.  Utilize as necessary interpreter language line and accommodation resources based on the university’s Americans with Disability Act (ADA) guidelines, such as American Sign Language (ASL).  Promotes interagency collaboration through entities such as HFS, DCFS, and other community or state agencies committed to the participant's care.  Educate, support, and connect non-waiver families with resources for a seamless age transition. Similarly, provide age-transition support to waiver families exiting the program due to health improvement.  Completes consistent and timely documentation (within 48 hours) to ensure compliance with waiver and non-waiver renewal requirements and timelines without direct manager support.  Conduct and document in-person visits at home or other appropriate settings like schools or hospitals every 6 months or as needed according to federal waiver requirements.  Identify/escalate and facilitate internal team meetings on participants with complex behavioral/social determinants or clinical factors impacting their well-being.  Identifies critical incidents and collaborates with all involved parts for resolution.  Active participation in post-records reviews and completion of recommended remediation within expected timeline.  Contribute to quality improvement initiatives, including but not limited to attendance at quality huddles and provision of recommendations as needed.  Arrange, lead, and contribute with areas of expertise to multi or interdisciplinary care team meetings with participants’ providers, family members, nursing agencies, or school teams.  Apply effective communication skills to improve families’ health literacy.  Manages clinically complex caseload participants resulting from neglect or abuse allegations, illness progression, or caregivers’ hardship.  May support other licensed and unlicensed care coordinators in verifying and interpreting clinical conditions, treatments, mental/behavioral health diagnoses or concerns, guiding priorities on the person-centered care plan, and recommending resources.  May mentor/coach care coordination team members and participants/caregivers on self-management of chronic diseases, medication adherence, and prevention.  Serves as a consultant for team members supporting families undergoing transitions of care. May contribute as a subject matter expert on health education initiatives such as immunizations, weight management, the importance of physical activities, etc. Assists families with private/public health insurance through effective benefits management practices for recipients. Complies with the University, Division, and Regional Office policies, and procedures.
