Summary: The Licensed Practical Nurse is responsible for providing nursing care for a defined group of patients according to the facility’s philosophy of patient care and standards of professional nursing practice. She/he reports up through the Charge Nurse and ultimately the Clinical Nurse Manager and is part of the treatment team. She/he will work collaboratively with other disciplines of the unit. Education: In addition to the skills normally obtained through completion of a High School education, must be a graduate of an approved licensed practical nursing program and maintain a current Rhode Island license. Licensure: Active LPN license Experience: Experience in behavioral health and/or skilled care preferred. BLS training. Skills and Abilities: Must possess excellent interpersonal skills, adaptability, and tact creating a favorable image in relationship to the supported department. Must visibly demonstrate a commitment to customer relations. Requires timely, positive responses to the needs of all customers including physicians, residents, families, and co-workers. Working Conditions, Physical Environment and/or Safety Requirements : Must possess sound dexterity and visual acuity. Requires mental alertness, stamina to work in a fast-paced environment, and in emergency situations. Ability to stand, walk, bend, and reach without restrictions. Ability to lift between 25 and 50 pounds using good body mechanics. Must be able to push heavy equipment, gurneys, and wheelchairs. May be exposed to all patients care elements within the facility setting. Risk of needle punctures. Must meet annual safety training requirements per facility policy. Some risk of electrical, mechanical, and chemical hazards. Will be required to work the hours for which hired including weekends and holiday coverage.

SUPPORTIVE CARE: a) Provides for the care of the residents’ personal belongings and the environment according to the plan of care including: • Preparing, washing and storing personal laundry according to the resident’s preference. • Making the resident’s bed. • Straightening the resident’s room and maintaining a clean and safe environment. b) Assists in the dining process by: • Documenting Meal Attendance for all meals in binder in the Memory Support Program. • Collaborating with the wait staff to monitor attendance at meals in the Assisted Living Program. c) Escorts residents to activities within the facility and actively participates with residents. d) Assists in the orientation of new residents upon admission and if the resident desires, assists with the unpacking of his/her personal belongings. e) Assists with the preparation of residents and their belongings for home visits, transfers or discharges. f) Observes residents and notes their physical conditions, attitudes, reactions, appetite, etc. Reports any changes in condition and/or unusual findings. QUALIFICATIONS: 1. Minimum of a High School Diploma or GED required. Completion of a Training Program for Home Health Aides. 2. Current Home Health Aide certification. 3. Previous experience serving the dementia or geriatric population preferred. 4. Ability to work with other staff members, residents, visitors and the general public. 5. Ability to organize and prioritize multiple tasks effectively.
