HOW YOU'LL WORK: You’ll help direct the activities of assigned community staff and provide sales related support to market/agency /program management to ensure market sales and profit targets are achieved or exceeded. MAJOR AREAS OF RESPONSIBILITY: Manage and oversee overall community growth and strategic plan for dedicated agencies. Work with region and agency leadership to implement appropriate sales structure and prioritize key customers in accordance with market strategic growth plan. Be responsible for day-to-day oversight and management of community marketing personnel and activities to meet/exceed assigned sales target. Provide support to community marketing personnel for dedicated agencies and serve as a point of escalation for questions and subject matter expertise. Conduct administrative oversight functions for community marketing personnel for dedicated agencies such as time off approvals and assessing additional support needs. Work with RVP, Home Health and VP Partnership and Community leadership to develop and execute market specific annual strategic plan/budget and prepare quarterly sales updates to ensure achievement of established sales targets and compliance with established budgetary constraints. Maintain a sales territory and call on physicians, multiple facility types such as hospital, skilled nursing, rehabilitation, facility discharge planning services and/or management, assisted living and other senior living settings within assigned territory. Maintain detailed territory list, prioritizing accounts in accordance with the market/territory Make sales calls, arrange appointments and deliver educational programs to keep referral sources current in local agency service offerings. Act as a liaison between referral sources and company staff to provide information on company services, resolve problems and maintain positive customer relations. Other duties as assigned. You will attend discharge/multidisciplinary rounds in acute care, ambulatory and/or other settings within the health system to share your expertise and to assist the patient in transition of care from one setting to the next within the health system. Patient Care: Coordinate health care services as ordered by the attending physician. Discharge Planning: Assist hospital/facility personnel in the discharge planning process. Minimize Patient Risk: Ensure coordination of all ancillary services per the patients’ needs following discharge. Promote well-being of patients. Customer Service: Increase awareness of services offered and service account(s) to maintain facility relationships. Build and maintain lasting positive relationships with patients/clients and facility/hospital personnel, physicians and other team members. Patient Advocate: Function as a resource nurse/social worker for your patients. Policies: Review and complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaboration: Participate in care conferences and coordination of case management. Notify the referring facility manager before contacting patients. Operations: Participate in Care Integration meetings.
