In accordance with regulatory requirements and the mission and vision of VNA of the Treasure Coast, this position is responsible for coordination and management of patient care and the supervision of clinical associates to ensure that care and services are delivered appropriately. Demonstrate Core Values in all duties. Perform duties under general supervision of Director or VP of Clinical Services. Per regulation, Clinical Team Supervisor (PCM) may functions as the Director of Nursing. Responsibilities Plan, organize, direct and evaluate clinical service delivery to continuously improve the quality, coordination, and effectiveness of service delivery. Understand and communicate policies, procedures and clinical protocols to assure that reporting staff have a working knowledge of agency practices and that care is delivered in accordance with agency expectations. In cooperation with the VP of Clinical Services or Director of Professional Services Home Health and the Regulatory Compliance Quality function, actively participate in the oversight of data collection (both internal and publically reported) measuring clinical performance and patient outcomes. The Patient Care Manager is responsible to contribute to corrective action plans when negative trends are reported. The Clinical Team Supervisor (PCM) utilizes this information in the completion of periodic shared field visits and annual performance appraisals Participate in developing programs to improve clinical, system and fiscal outcomes. Demonstrate working knowledge of Medicare/Medicaid Conditions of Participation and Joint Commission accreditation standards applicable to home health. Demonstrate working knowledge of the managed care industry. Ensure compliance with applicable legal requirements, standards, policies and procedures. Supervise associates in the home health nursing department and/or team. Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training; planning, assigning, and directing work; appraising performance; rewarding and disciplining associates; addressing complaints and resolving problems. Demonstrate clinical and management skills including hands-on care, nursing judgment, organization, leadership, problem solving, critical thinking, verbal/non-verbal communication and conflict resolution Mentor nurses in development of their management skills i.e.,critical thinking, time management, and problem solving. Effectively use the agency’s software programs to plan, coordinate and deliver patient care including processing physician orders, plans of treatment, scheduling, review and oversight of OASIS and other applicable assessment tools Work collaboratively with other departments in the coordination of care and services. Qualifications Current RN license in the State of Florida Minimum of three (3) plus year’s experience as an RN including management experience and (1) year in home health. #indspn

This position will oversee patient case load while coordinating all patient services with members of interdisciplinary team. This position will perform initial patient evaluation assessment to identify nursing diagnosis and treatment of care in order to meet the needs of the patient. Assess patient, living environment and family as consideration when creating and executing care plan. Will establish interventions and interface with physician as needed. Responsibilities Assess client environment including family support to develop and establish care plan for patient, education about care and treatment for patient, utilizing family members or designated persons Conduct initial assessment (admission) and establish plan for treatment to obtain thorough physical, psychosocial and environmental data; establish and follow a care plan which identifies problems (nursing diagnosis) and evaluate outcome within a certain time frame Conduct on-going assessment of patient response to treatment, medication and teaching with intervention as needed. Revise care plan as needed Provide direct patient care and facilitate focused communication with client and family, establishment of the nursing care plan, interventions, and evaluate outcome Coordinate all services and collaborate with the interdisciplinary team as needed Communicate with physicians to confirm, correlate, update patient’s plan of treatment and provide physician with a status report on a regular basis. Maintain and update clinical records, according to agency policy, State and Federal guidelines. Complete paper and electronic documentation daily and timely to meet agency expectations. Responsible for scheduling clients on a weekly basis, updating as needed. Provide instruction and supervision of LPN's and HHA's on a timely basis; and in accordance with agency, State and Federal regulations. Must work a minimum of one weekend a month. Required to work one summer holiday and one winter holiday. Qualifications Current RN License in the State of Florida At least one year of home health experience Must have working knowledge of home care home base- point care Must have working knowledge of Oasis documentation. Coordination and work with the Education department and give educational and informational presentations during staff meetings regarding system enhancements. Responsible for maintaining current knowledge of application updates and changes. May be determined that home visits with clinical staff to teach and assess proficiency on Point of Care devices. Utilize a team approach to plan and oversee departmental computer orientation to reflect current practice and regulatory compliance standards using current operating systems. Assess the orientee’s ability to meet basic performance standards. Facilitate the development of clinical staff in the role of learning new/basic computer skills and information management. Use computer technology to distribute educational content and evaluation tools to staff through a variety of computer applications (i.e. E-mail, Intranet, and Internet). Participate in opportunities for professional development that enhance the level of knowledge and skill related to the profession of Clinical Informatics. Maintain a level of competence required to meet the current and future needs of the organization by keeping abreast in current literature and courses. Qualifications Bachelor’s Degree, nursing or other healthcare-related field and / or three years of direct patient care experience combined Clinical knowledge, including Oasis and Coding Certification Advanced experience and/or education in computer applications. Knowledge of Homecare Homebase software a plus. Two years clinical nursing or other healthcare professional experience. Current home health and/or hospice experience, including knowledge of PPS, State, Federal, and JCAHO regulations. Proven experience with IT applications and teaching. Extensive knowledge of home health and/or hospice care operations. Ability to perform complex, detailed design and analytical tasks. Excellent oral, written and interpersonal communication skills as well as professional appearance. #indspn
