Registered Nurse (RN) Hospice Case Manager

Overview R egistered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager plans and manages the nursing component of the hospice caseload as well as the patient's illness and symptoms. He/She re-evaluates effectiveness of interventions and adjusts the care plan as needed. Overview Registered Nurse (RN) SIGN ON BONUS - Hospice Inpatient Unit – The RN-IPU initiates and maintains the care plan for patients in the acute care setting based on decisions and wishes that have been established by the patient and their families. The RN-IPU focuses upon symptom management while addressing all other healthcare needs. The RN-Hospice IPU works closely with the in-patient team to provide support to patients and their families. Overview Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager manages symptoms of the patient's disease and re-evaluates effectiveness of interventions and adjusts the care plan as needed. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs and wishes. Location: Falls Church, VA Hours: Monday-Friday 8:00AM -5:00PM with occasional coverage in the holiday rotation COVID-19 vaccine required to be completed upon start. Responsibilities The Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments. Qualifications Experience Requirements Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred. Education Requirements Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Required Certificates and/or Licenses Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.

Overview The Registered Nurse (RN)-Nights & Weekends is prepared to handle all on-going needs, in addition to urgent needs of patients and their families after hours in their homes or facility. The RN Nights & Weekends follows the established care plan of the patient while ensuring comfort care to the patient and support to family members. Location: Falls Church, Arlington, Alexandria Schedule: Every other weekend days , some nights and some holidays. COVID-19 vaccine required to be completed upon start. Responsibilities The RN Nights & Weekends visits patients with emergent, critical needs in a timely manner. The RN Nights & Weekends arranges for, teaches, and supervises nursing care of patients in home care and facility settings in collaboration with other members of the interdisciplinary team.
