Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. The CR nurse coordinates patient activities, provides coaching and education, communication with referring provider, and safety oversight during medically supervised exercise. RN Supervisor (Registered Nurse) GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Assure that residents have a clean, safe, orderly and comfortable environment. • Initiate resident Plans of Care on admission. • Assist in implementing recommendations from consultants and the Quality Assurance team as directed by the Director of Nursing Services. • Assist in developing, reviewing, revising an updating resident Plans of Care for individual residents. • Ensure that notification is given to the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within our established policies. • Review complaints and grievances form families and notify the Director of Nursing Services as necessary. • Evaluate emergencies and assist in implementing emergency measures. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform nursing care. • Assist in notification of physician of any resident incidents/unusual occurrences. Ensure that the Accident/Incident Report forms are filled out and completed on all such occurrences and such information charted in the resident’s medical record as outlined in our established policies and procedures. • Assist the Charge Nurse on investigation of alleged abuse or unusual occurrence as outlined within our established policies and procedures. • Start and monitor intravenous medication as ordered. • Assist the Charge Nurse and other nursing personnel in performing nursing procedures as necessary. • Assist the Charge Nurse in admitting, transferring and discharging residents. • Assist in correcting findings and deficiencies from the Department of Health Services, Consultants, and the Quality Assurance Team. Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
