After competency verification, the Licensed Practical Nurse may deliver patient care services to the adult, and geriatric populations. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, dressing, grooming, toileting, ambulating, transferring, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Collects and records intake and output. Prepare and administer oral medications as ordered by the physician or APC. Review medication history with patient. Assist with minor medical/surgical procedures in the practice setting. Performance of venipuncture for the purpose of blood and to monitor the patient’s condition before, during and after the withdrawal of blood. Transport patients using wheelchair or stretcher within offices and to/from vehicles. Assist those patients requiring help with ambulation. Identify the need for patient education and advise patients on their treatment plan. Demonstrate the ability to recognize abnormal lab results. Document treatment records for each patient. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents. Set up for suturing and procedures, using sterile technique; dismantles procedure set, caring for soiled instruments safely and correctly. Maintains competency in specialized testing for Occupational Health (Hearing Booth, Fit testing, Titmus, PFT, Ishahara, Drug screen collections, Breath Alcohol Testing, DS Hair Collections and any other required testing.) Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice. Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medications. Re-order supplies as required and within budgetary limitations. Maintain work station cleanliness and restock linen and supplies to exam rooms; provides general cleaning, including emptying trash, straightening waiting area, etc. Washes and changes stretcher or exam table linen and empties dirty laundry. Demonstrate the ability to reflect knowledge of current professional practice standards, laws and regulations, evaluate their own nursing practice in relation to professional practice standards, relevant statutes and regulations. Engage in an annual self-assessment, identifying areas of strength and areas requiring professional development Seek and reflect on constructive feedback from other members of the healthcare team. Acts to achieve performance goals. Acquire and maintain current knowledge and competency in nursing care. Orders and collects data using appropriate evidence-based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Prescribes diagnostic strategies and therapeutic interventions. May perform advanced procedures consistent with privileges and competency validation. Communication and Collaboration Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care. Documentation Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, in medical record per specific patient unit or service, death summary) Documentation is timely, meets acute care compliance standards and captures patient acuity. The RN will function at the top of their license in areas of leadership, delegation, initial patient education, triage of patients, precepting new staff, mentorship, and behaving as a role model for other clinical team members. The RN functions as a liaison between clinical team members and leadership to develop office specific policies, procedures, and/or protocols to promote a safe working environment and efficient workflows. Responsibilities Triages patients and uses clinical judgement to implement nursing interventions in a timely manner. Delegates elements of care to appropriate healthcare workers in accordance with the Nurse Practice Act and the State of Delaware Board of Nursing Rules and Regulations. Acts as a preceptor and mentor for other licensed and unlicensed clinical team members. Timely completion of assigned education and annual competencies. Develops, implements, and evaluates an identified plan of care. Prioritizes and coordinates care delivery and provides initial education to patients, families, and/or caregivers. Administers medications according to policy. (IV push medications may only be administered by RNs per the Nurse Practice Act and the State of Delaware Board of Nursing Rules and Regulations) Collects, analyzes, and documents the assessment data to determine the actual or potential nursing diagnoses, problems, and identifies expected outcomes. Recognizes unit needs and is a liaison between the clinical staff and leadership to assist in development and maintenance of office specific policies, procedures, and/or protocols that will promote safety and encourage efficient workflows. Communicates and collaborates with multidisciplinary teams and other key stakeholders in the conduct of nursing practice. Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible. Advocates for clients. Actively assists others and accepts additional duties or assignments. Works in other BMG office locations and/or other shift times when needed. Qualifications Education: RN Diploma RN Degree Program Essential Credentials: RN - Registered Nurse license from the state of Delaware or Compact State BLS - Basic Life Support (American Heart Association) Education Other Information Entry USD $26.12/Hr.
