Be the Best Hospice Aide You Can Be If you meet these qualifications, we would love to meet you: Certified by the state as a nurse’s aide, with completion of required training and a passing score of 85% or higher on the agency’s competency exam; additional requirements vary by state (e.g. CHHA in CA/NJ, CGNA in MD). CPR certification required for direct patient care in Georgia and Washington. Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals. Requires a reliable vehicle, valid driver’s license, and state-required auto insurance. Responsibilities: Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance). Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care. Monitor comfort levels, respond to needs, and assist with daily living activities, always maintaining patient safety and dignity. Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols. Provides direct care, administers treatments and medications, organizes and distributes daily assignments to direct care staff consistent with each individual resident's comprehensive resident assessment and plan of care. Supervises direct care staff and makes decisions about resident care needs consistent with facility policies and procedures. Ensures that “Flow of Care” is followed DUTIES: Knowledge of geriatric and general chronic care nursing principles and professional standards of nursing practice and ability to apply to resident specific circumstances. Able to identify, implement and evaluate appropriate objectives and interventions for residents Makes daily compliance rounds on units at least at beginning and at end of shift to observe, examine or interview residents, to verify information from ongoing shift, to monitor regulatory compliance, to determine staff assignments, and provides/receives report Monitors delivery of care and services to ensure needs are met, tasks are completed and that work of direct care staff is of acceptable quality and quantity Demonstrated competency in clinical skills needed for facility resident population. Demonstrated competency in collecting data of physical, behavioral and functional ability of long term care residents. Working knowledge of laws and regulations that influence provision of care and services in nursing facilities. Working knowledge and ability to interpret and demonstrate facility policies and procedures to direct care staff. Sufficient organizational and interpersonal skills to assign, motivate staff, elicit work output, improve quality and interact effectively with residents, families, interdisciplinary team members, administration, government officials, consultants and others in addition to staff. Carries out all duties in accord with the facility mission and philosophy. Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions. Appreciates the importance of maintaining confidentiality of resident and facility information. Demonstrates honesty and integrity at all times in the care and use of resident and facility property. Assumes responsibility and accountability for nursing services delivered to residents Provides direct care Administers treatments and medications Organizes and distributes daily assignments to direct care staff consistent with each individual resident's comprehensive resident assessment and plan of care Supervises direct care staff and makes decisions about resident care needs consistent with facility policies and procedures Ensures that “Flow of Care” is followed.
