BEHAVIORAL HEALTH RN I (Full-Time Nights)

JOB RESPONSIBILLITIES: Restores and promotes patient’s health by completing the nursing process including but not limited to: a. Completes initial nursing assessment within 24 hours of admission or per Behavioral Health Unit/JMH (Johnson Memorial Health) policy; reassesses routinely throughout patient’s stay on the unit. b. Completes individualized treatment plan for each patient. c. Plans, implements, and coordinates nursing care activities for patients. d. Conducts therapies in accordance with patient’s diagnosis and needs. e. Performs treatment interventions following physician orders such as medication administration and medical treatment orders. f. Ensures patient’s dignity and respects patient values. g. Assists with patient personal needs (oral hygiene, toileting, bathing, dressing, transferring, etc.) as required. h. Operates and participates in tele psych. i. Draws patient blood as ordered. Collaborates with physicians, multi-disciplinary team members, and outside health and social service agencies. Interprets the nursing and treatment needs of the patient for nonclinical staff assigned to the patient. Provides efficient discharge plans and obtains accurate histories at admission. Completes all required documentation relevant to patients in a timely, efficient manner. This includes but is not limited to: a. Admission paperwork b. Assessments (mental and physical) c. Progress notes d. Interventions e. Evaluations f. Discharge paperwork g. CQI+ data Protects patients and employees by adhering to safety standards, infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations. Uses hygienic practices and teaches patient hygienic practices. As needed, and using approved methods, assists in safely placing patients in seclusion or restraints and observes suicide/seclusion precautions including physical requirements for physical holds and safe transport of patients. Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs. Plans and implements nursing groups. Completes all required nursing staff competencies per program regulations in a timely manner. Orients or mentors new nursing staff as needed. Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and regulatory standards. Conducts patient chart audits. Establishes a compassionate environment by providing emotional and psychological support to patients and their families and friends. Participates in individual, group, and family psychotherapy and educational seminars as appropriate. Performs medication reconciliation. Ensures operations of equipment by completing preventive maintenance requirements, calling for repairs and evaluating new equipment and techniques. Provides feedback to supervisor on an ongoing basis regarding concerns, improvements, changes, etc. Maintains patient privacy and confidentiality and protects operations by keeping patient and program information confidential. Operates within ethical standards. Follows voluntary and involuntary procedures/processes. Performs other related duties as assigned. Emergency Nurse Pediatric Course or PALS must be completed within one year of hire. Maternity Center: ACLS or ALS Certification required. NPR certification is required within 3 months of hire. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Complete & file required recordkeeping forms/charts upon the resident’s admission, transfer and/or discharge. Receive & transcribe telephone orders from physicians & record on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Fill out and complete accident/incident reports & submit to Director as required. Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures. • Prepare & administer medications as ordered by the physician. • Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary. • Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, etc., as required. • Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. • Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident’s room. • Provide IV & IV site care as needed. • Performs other duties as assigned. QUALIFICATIONS: • Registered Nurse with current unencumbered state licensure. • Licensed Practical Nurse with current unencumbered state licensure. • Must be able to read, write, speak, and understand the English language. • Must be able to make independent decisions when circumstances warrant such action. • Must be knowledgeable of nursing & medical practices & procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. • Current CPR Certification is required • Experience in Long Term Care is preferred.
