Completing patient intake, preparing exam rooms, rooming patients, assisting the clinician, performing lab tests, providing injections, ordering and maintaining supplies, and variety of task which require demonstrating a knowledge of universal precautions, sterile techniques and hazardous waste disposal. Minimum Qualifications: High school diploma or equivalent. National Medical Assistant Certification Preferred Qualifications: 1-2 years in a healthcare setting. NP will be responsible for following up on all the remainder of their patients on their case load for the day and document the progress updates. NP will put in admission orders or review and make corrections as per their evaluations, will order baseline and follow up laboratory blood work as deemed necessary and follow up on reports and communicate with patient and family. NPs are expected to attend daily team rounding meetings on the unit in person Mon-Fri. NP’s will provide at least 1 to 2 nights of call coverage on pager depending on the weekend that they may be on call. NP’s will provide in person 24 coverage on Sat and Sun at least once every 5th week and will have the same responsibilities however the case load on the weekend will be a maximum of 12-13 patients and the rest of expectations for admissions, progress follow ups and discharges will remain the same as on weekdays. NP’s will be responsible for collaborating with ancillary staff members involved in their patients care ie nursing, social workers and mental health technicians. NP’s will be responsible for keeping up with all licensure requirements and update on their workday profile. Partners with EHMG operations teams to execute against care, quality and financial measures outlined in governmental, commercial, ACO and other FFS/enhanced FFS/Incentive based contracts. Responsible for the production and dissemination of various reports, dashboards, insights, and information to drive performance improvement associated with EvergreenHealth Medical Group Clinics, including Preventive Health and Health Maintenance initiatives. Responsible for tracking performance across various contractual models (e.g., MACRA,Medicare Advantage, etc.) and coordinating Clinical Integration, EHM Finance, IT, and administrative stakeholders to identify and address opportunities for improvement. Responsible for maintaining and reporting EHMG governmental quality metrics. Responsible for maintaining and reporting EHMG commercial insurer quality metrics. Coordinates production of quality-related reports, analyzes the data to identify opportunities for improvement, supports leaders and staff to develop appropriate improvement/risk mitigation plans. In collaboration with leadership, identifies improvement priorities and targets, sets annual goals, and facilitates improvement work as needed. Maintains current knowledge of public reporting initiatives and national and local clinical improvement initiatives that pertain to the EvergreenHealth Clinics. Decision-making regarding appropriateness of performance improvement methods, implications of data (including trends, variation, and best practice), and need for changes in process or practice. Works with others in a coaching and facilitator role to ensure the effectiveness of the performance improvement process. Organizes meetings, projects, and reports of various improvement activities. Coordinates the ongoing review, update and communication of quality related polices, procedures, and processes in support of achieving outstanding clinical outcomes and absolute safety. Performs other duties as assigned. License, Certification, Education or Experience: REQUIRED for the position: Bachelor’s Degree or equivalent combination of education and experience. 5 years of increasingly responsible quality and operations experience in a healthcare setting Previous experience working with quality management. Ability to use quality and quantitative data. Knowledge of quality improvement practices and methodology. Experience with quality performance measures, such as HEDIS, STARs, and patient experience measures. Experience with risk adjustment models.
