About the unit Our sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support. Our sub-acute unit offers a well-rounded opportunity for learning, professional growth and personal satisfaction. Our Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals. Our Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. The dementia care is where we create a fullness of life through the joy of relationships, the art of caring and the spirit of living. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care. About the Facility Corewell Health Commons – Farmington Hills Our facility has 3 floors and 9 unit (3 units per floor), we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds which focuses on long-term care and sub-acute rehab and our third floor is a secured dementia unit. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and sub-acute rehab residents. With 179 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate in multiple activities. Our goal is to create a Fullness of Life through the joy of relationships, the art of caring and the spirit of living. Scope of work Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team. Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit. Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding. Documents care provided as directed in a manner consistent with guidelines. About the unit 5 South is a cardiac stepdown or a telemetry unit. We have 32 beds with 28 semi-private rooms and 4 private rooms. Each patient has a telemetry monitor that they are required to wear at all times while on the unit. 5 South thrives on teamwork, kindness, and respect from all team members.
