How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Additional Details We are looking for a PRN LPN to join our clinical team at Lake County Jail. This role will cover the day shift, 7a-3:30p and evenings, 3p-11:30p We look for our PRN staff to work at least 4 shifts per month. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Must possess current CPR certificate or become certified within two weeks of employment. Must be knowledgeable of nursing and medical practices and procedures. Demonstrates interest and/or knowledge in care of the aged. Must be able to read, write, and speak the English language in an understandable manner. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with customers, family members, visitors, personnel, government agencies/personnel, and the general public. Must possess leadership and supervisory ability and willingness to work harmoniously with other personnel. Must be personable, enthusiastic, and resourceful. PHYSICAL REQUIREMENTS Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must function independently, have flexibility, personal integrity, and the ability to work effectively with customers, family members, personnel, and support agencies. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people. May be required to lift, push, pull and move equipment, supplies, and customers in excess of fifty (50) pounds throughout the day.
