Writes appropriate orders related to pre-existing conditions. d. Discusses with the attending physician and nursing staff physical and/or emotional findings, which may interfere with care of the patient. e. Manage all aspects of the patient’s symptoms, including medication/prescription management, conduct family meetings as needed and requested, coordinate care with other members of the IDT through daily rounds, and document care activities and the rationale for the GIP level of care. f. Responsibility for completion of the MOLST form as required and for the completion of the death certificate of each expired patient. g. Attend the weekly inter-disciplinary team meetings h. Work with the hospice access team to facilitate a smooth admission process. Work with the Gilchrist Inpatient Centers inter-disciplinary team to facilitate live discharges from the Center, including completion of transfer/discharge summary, prescriptions, and other discharge related documentation. i. Provide telephone and afterhours coverage via emedicall for patients at the assigned Center Evaluates patient progress on a continuing basis and in collaboration with the attending physician, and alters treatment modalities as needed. a. Registered Nurse (RN) Per Diem ALL SHIFTS The Registered Nurse is responsible for the management of an assigned unit on a specific tour of duty. ESSENTIAL REQUIREMENTS: Must currently be licensed as a Registered Nurse by the PA State Board of Nurse Examiners. Must be knowledgeable of nursing, medical practices and procedures. Must be knowledgeable of State and Federal Regulations for Long Term Care. Employment is contingent on completion of all pre-employment checks inclusive of a criminal background check and Medicaid Exclusion Screening, both indicating negative results. ESSENTIAL DUTIES AND RESPONSIBILITIES Know and support the philosophy, goal and objectives of the Nursing Department. Supervise Nursing Assistants and Staff Assistants. Assign resident care responsibilities to nursing personnel based on residents' needs, available staff and unit needs. Collaborate, communicate and cooperate as appropriate with other health care providers to insure quality care, including transcribing physician's orders according to facility procedures. Continually observe/monitor and evaluate quality, quantity and timeliness of nursing care. Deliver treatment modalities according to physicians orders Implement and update care plans as appropriate Ensure documentation is appropriate and accurate at all times Assess need for modification of medical regimen and/or nursing care plan and ensure the clinical records are properly maintained. Direct the delivery of care using sound good judgement while applying the highest standards of care and within the nurse practice act. Provides for and assesses the most complex resident needs to ensure appropriate care Teaches appropriate nursing techniques to residents, staff and/or family. Administer medications and treatments according to physician’s orders and established policy and procedures. Report falls, unusual occurrences and significant changes in resident's condition to the supervisor, physician, caseworker and responsible party according to facility policy and procedures. Initiates and when appropriate completes investigations of resident grievances, incidents, elopements, falls, abuse allegations, and any other unusual incidents that require an investigation. Document according to requirements of facility and maintain strict confidentiality. Give appropriate report in a professional manner to the nurses on next shift according to requirements of facility. Ensure established inventory standards and proper maintenance of equipment is complete. Seek consultation as needed from supervisor. Assist the Unit Manager in conducting team conferences to assist in planning nursing care, problem-solving techniques for unit problems and evaluating staff's involvement and attitude. Attend continuing educational programs relating to responsibilities of the position and professional growth. Act as role model for nursing assistants, licensed staff and other personnel. Maintain and enforce department and facility procedures, fire prevention and safety standards Must be an active participant in the facility’s Quality Assurance/Performance Improvement (QAPI) program and Performance Improvement Project (PIP) and carry out roles and responsibilities as assigned. Must attend trainings for QAPI, follow policies and procedures in support of QAPI efforts, and support QAPI initiatives and findings. Must also report potential or actual non-compliance issues to the facility’s QAPI committee. Comply with Health Insurance Portability and Accountability Act (HIPAA) regulations. Responsible for evaluating work performance evaluations of nursing assistants and those in a direct subordinate role. Responsible for disciplinary action including suspension and termination of nursing assistants when appropriate. · Will possibly be subject to exposure to infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses. Perform other reasonable duties as assigned by supervisor.
