Nurse Practitioner

Monday through Friday 8 to 5pm, no nights, weekends, or holidays! The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Evaluates participants and documents information in participant’s chart. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records. In-Home Services CNA - Contract Opportunity

The In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Documents care provided in accordance with established policies, guidelines and applicable regulations Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Participates in the care planning of participants through cooperation with the multidisciplinary teams. Monitors participants’ health status by taking vital signs and weights (monthly). Monitors appetite in addition to looking for changes in functional ability and checking skin condition while assisting with toileting. Monitors/assists participants in a courteous manner with meals and provides assistance with feeding as necessary. Assists participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitors participants’ health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reports pertinent information to clinical supervisor in a timely manner. Reports any variance, accident or unusual occurrence to the appropriate personnel. Attends in-service and continuing education programs as scheduled and necessary. GENERAL STATEMENT OF RESPONSIBILITIES: This is a FLSA non-exempt position. The incumbent serves as a Certified Nursing Assistant in a Day Support Program for adults with intellectual disabilities. Services are provided to individuals with mild, moderate, severe or profound intellectual disabilities. The CNA must maintain safe conditions for the program participants and provide specific training, supervision, and assistance services, documenting as directed. Responsibilities 1. *Ensures the health and safety of consumers and program participants and provides planned and emergency services as described: a. physically assists, supervises and/or trains all consumers in personal care skills (i.e., toileting, feeding, showering/bathing, tooth brushing, topical medications, menstrual care) as needed. b. assists with assessment and delivery of health care needs including: the self-administration of consumer medications, BSL checks, insulin, blood pressure checks, physical therapy as defined in medical care plan c. performs CPR/first Aid/crisis interventions for medical or behavioral emergencies following training guidelines d. counsels and manages behaviors by prevention or de-escalation of aggression Uses positive programming methods to address behavior needs in accordance with licensure regulations and Human rights guidelines. 2. *Assists directly in the implementation of consumer's individual program plans. 3. Supervises the LPN every 30-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel.
