Principal Duties/Responsibilities: Provides ongoing supervision of residents’ health related activities; complying with health care instructions explained by health care professionals and promptly and appropriately seeking and obtaining medical attention when needed Recognizes and can assist in the prevention of elder abuse, neglect, and exploitation Recognizes and reports a resident’s change of condition particularly as it relates to needing more time to deliver care Assists residents with Activities of Daily Living (bathing, grooming, hygiene, dressing, toileting, mobility/transfers, dining) Participates in functional evaluation and service plan reviews Removes, launders, folds, and properly returns resident linens and personal laundry Serves residents food as directed, including feeding skills, if regulations permit Receives and gives report of on-going shifts by oral and/or written log Effectively handles resident behavioral expressions Reacts calmly and effectively in emergency situations Assures resident information is communicated to the staff through the report process and calendar (i.e. appointments, outings, etc.) Efficiently documents and provides accurate record-keeping as appropriate Checks all residents daily for cleanliness, appropriate clothing, and any continence issues Uses proper sanitary procedures to wipe tables, and sweep and mop dining room and life enrichment kitchen after meals Checks on resident rooms daily for general cleanliness and neatness; ensures high risk rooms are double checked Communicates and documents residents’ material needs (i.e. briefs, clothing, toothpaste, pads, etc.) Measures and accurately records vital signs (temperatures, pulse, respiration, blood pressure, weight and height) where appropriate as per state regulations Accurately communicates and documents residents’ status Assesses resident safety/risk in a proactive manner Effectively communicates with residents, families and friends of residents Accurately completes incident reports prior to the end of the working day Participates with Programming staff and residents in daily programs and recreational activities as directed. This position will have a combination of work out of our Travelers office and work out of a Concentra clinic in Manchester, NH. Under moderate supervision, provide office based telephonic and/or in-person medical case management supporting Concierge locations with emphasis on early intervention, return to work planning, coordination of quality medical care on claims involving disability and medical treatment as well as in-house medical reviews as applicable to claim handling laws and regulations. Responsible for helping to ensure injured parties receive appropriate treatment directly related to the compensable injury or assist claim handlers in managing medical treatment to an appropriate resolution. What Will You Do? Meet with injured employees face to face following office visits at a provider's medical facility to assist with the claim process and ensure compliance with their medical treatment plan. Contact customer, medical provider and injured parties on claims involving medical treatment and /or disability to coordinate appropriate medical care and return to work. Develop strategies to facilitate an injured employee's return to work and achieve maximum medical improvement. Evaluate and update treatment and return to work plans within established protocols throughout the life of the claim. Coordinate with medical providers to ensure the injured employee is actively participating in a viable treatment plan. Evaluate medical treatment requests to ensure that they are reasonable and necessary based upon jurisdictional guidelines. Engage specialty resources as needed to achieve optimal resolution (Dial-a-doc, physician advisor, peer reviews, MCU). Partner with Claim Professional to provide medical information and disability status necessary to create an overall strategy to achieve an optimal outcome. Utilize internal Claim Platform Systems to manage all claim activities on a timely basis. Utilize Preferred Provider Network per jurisdictional guidelines. Partner with Claim Professional to provide input on medical treatment and recovery time to assist in evaluating appropriate claim reserves. Submit accurate billing documentation on all activities as outlined in established guidelines. Customer Engagement. Participate in Telephonic and/or onsite File Reviews. Respond to inquiries - oral and written. Keep injured worker apprised of claim status.
