Assists the RN in gathering assessment data, providing skilled nursing care as well as instruction/education to hospice patients and families as indicated. The Hospice LPN works closely with the RN and other team members, performing/assisting with procedures as appropriate and/or directing Hospice Nursing Assistants for further care as necessary. Collaborates with physicians and other health care practitioners, obtaining and implementing orders as needed. Documents all care provided in accordance with established policies/ procedures, updating Plans of Care as needed to reflect interventions performed. The Hospice LPN will have primary assignment to either Home Care or Inpatient Care, but is expected to help in areas of need within any division/department of HOSG as directed by the Nurse Manager. May assist with medication and supply pick up as needed, as well as visits to clients in the home care setting as directed. Represents HOSG and its services in a competent, professional manner consistent with the organization’s culture of service and teamwork. KNOWLEDGE, SKILLS & ABILITIES Required current licensure as an LPN in the State of Georgia with preferred certification in Hospice and Palliative Care Preferred minimum of two (2) years varied work experience as a Licensed Practical Nurse Demonstrated understanding of and adherence to the hospice philosophy and ability to work as member of interdisciplinary team in meeting the unique needs of the terminally ill Must possess strong clinical skills in assessment, intervention, documentation, and evaluation Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with members of the hospice team, physicians and other referral sources, patients/families, and community agencies/institutions Must possess ability to work non-judgmentally with persons of diverse cultural, religious, and socioeconomic backgrounds and lifestyles Must be self motivated with strong organizational skills and demonstrated ability to prioritize tasks, handle pressure, and utilize critical thinking/problem-solving techniques Demonstrated ability to respond well to supervision and work independently with minimal direction and appropriate collaboration when required Ability to operate clinical equipment as well as office equipment such as computers, telecommunications devices, etc. Must have current Georgia driver’s license with satisfactory driving record and the availability of a reliable, licensed and insured vehicle for use on the job Must have current CPR certification WORKING CONDITIONS - ADA INFORMATION Work environment may include hospice office, patient homes, nursing homes, and/or inpatient hospice unit with primary assignment to either Home Care or Inpatient Care Environment. Expectation is to assist in either area as directed by Nurse Manager/RN. Generally assigned to a consistent shift with regular working hours according to assigned department. May require long, irregular, or flexible work hours including some weekend, on-call, evening, and/or holiday hours on either a scheduled or rotating basis. Periods of moderate to high levels of stress and emotionally difficult situations. May require considerable mental effort in dealing with persons who may be under stress. May include travel to patient’s homes, pharmacy, hospital, and other care settings. Occasional overnight and/or out of town educational workshops, approximately 1-2X per year. Physical work may require sitting, standing, bending, and frequent lifting or pushing objects (excess of 50 pounds), using correct techniques. May require extensive standing and walking. Likely to be exposed to blood, body tissue and fluids as well as other hazardous waste materials on a daily basis. Ability to operate basic medical equipment as well as office machines such as telephone, fax, copier, and computer required. May be requested to assist with referrals/admissions or other duties as directed by the Nurse Manager, RN or Executive Director during periods of high census or staff illness/absence.
