RESPONSIBILITIES The RN Case Manager will: Perform initial and ongoing comprehensive nursing assessments for hospice patients. Provide compassionate clinical care, including symptom management, treatments, and preventive nursing procedures. Deliver dietary counseling and patient/family education to support comfort and quality of life. Observe and promptly report significant changes in patient condition to the physician and interdisciplinary team. Collaborate with the IDG to develop, update, and implement individualized care plans. Teach, supervise, and participate in the ongoing training of nursing personnel. Maintain clinical documentation in compliance with Harmony Hospice standards, Medicare and HIPAA regulations. Participate in on-call rotation to ensure 24/7 patient support. Meet agency infection control, quality, and compliance standards. Attend required meetings, team conferences, and continuing education. Perform additional duties as assigned by the Director. Refill prescriptions, order supplies and DME, and coordinate services for patients and families. Document all calls, conversations, and outcomes in the EMR system. Receive start-of-shift reports and provide end-of-shift reports according to workflow. Perform initial nursing assessments on new admissions. Complete, coordinate, and revise patient Plans of Care in collaboration with the care team. Maintain accurate, timely, and compliant clinical documentation. Communicate changes in pain/symptom management. Educate families on patient condition, needs, and care expectations. Make referrals to other hospice disciplines as needed and document rationale. Deliver on-call responsibilities and services as assigned. Perform other related duties as needed. Requirements Must be a Registered Nurse with hospice experience preferred; telephone triage experience a plus. Active RN license in good standing with the State of Ohio. Strong understanding of hospice philosophy and the unique needs of terminally ill patients. Flexible availability for after-hours, weekend, and on-call schedules. Strong computer skills and ability to navigate EMR and software systems. Ability to handle stressful situations and support grieving families.
