How you make a difference The Clinical Coordinator MAT provides leadership, vision, oversight, and coordination to the provision of medication assisted treatment services to include program development, implementation, and supervision of the Medication Assisted Treatment initiative within the assigned area. Key Responsibilities Plan, manage, and supervise MAT services for patients in assigned facilities, fostering collaboration among medical, nursing, mental health, psychiatry, and security teams. Assist in program design and expansion of MAT services, including staffing and program development, in coordination with the Regional Office. Provide clinical supervision, professional education, and staff development for on-site service providers. Direct clinical services according to licensure and scope of practice standards. Represent the corporation's interests, ensure cooperation with facility administration, and oversee training provided by healthcare staff to other facility personnel. Qualifications & Requirements Education Minimum of Independent Licensure with a Master’s Degree in clinical/counseling psychology or social work from an accredited college or university or be a graduate from an Accredited School of Nursing with a current licensure as a Registered Nurse (RN) in the state in which employed. Other substance abuse certification would be desirable though not required. Experience Experience in the coordination and administration of behavioral health or medical service delivery systems as well as knowledge and/or interest in the provision of integrated behavioral health/medical programming within a multidisciplinary framework. Professional experience in the behavioral health or medical field that indicates knowledge of management and supervision techniques, peer review, data collection and quality assurance. Experience successfully navigating a correctional healthcare setting in which there are multiple stakeholders and needs to consider while maintaining the patient’s needs as the primary focus. Licenses/Certifications Related certification may be applicable based on specific state requirements. About this role The Chronic Care Nurse (RN) is responsible for managing and providing timely care to patients with chronic medical conditions by working closely with on-site physicians and mid-level providers. This position ensures that chronically ill patients are seen on a routine and scheduled basis according to their disease process. The Chronic Care Nurse (RN) also tracks and manages the overall care of these patients. What you bring to the table Education Must be a graduate of an accredited school of nursing Experience Three (3) years of relevant clinical experience is required. Prior medical/surgical or emergency/trauma experience is preferred. Previous corrections experience preferred. Licenses/Certifications Currently licensed as a Registered Nurse in the state within which services are provided and possess and maintain current CPR certification. What you will do Maintain a comprehensive patient log and schedule for chronic care patients. Assess patients as directed by physicians or mid-level providers and prepare patients for visits, ensuring all required diagnostic testing is complete. Provide patient education, address special needs, schedule off-site consultations, and ensure off-site recommendations are presented to providers. Maintain chronic care statistical data, review aftercare recommendations, conduct medication compliance studies, and maximize use of facility services. Ensure compliance with National Commission on Correctional Health Care standards for chronic care programs. How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities • Respond to and deliver care according to protocols and procedures.• Evaluate and treat patients for acute care and emergency medicine complaints based on standardized procedures and protocols.• Consult with a physician for conditions outside protocol parameters and identify patients who require transfer to an Emergency Department.• Assess patients' medical, traumatic, and psychiatric issues and document all findings using the S.O.A.P method and EMR.• Participate in Quality Management and Peer Review activities, attend administrative meetings, and provide clinical support for other services as needed. Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. Scheduling flexibility is also required to be able to rotate hours and shifts, if needed, and to be called during emergency situations to provide coverage. Licenses/Certifications Must have and maintain current licensure as a Nurse Practitioner within the state of employment. Must have and maintain current CPR certification.
