MAIN FUNCTION: Provides nursing care to patients in an efficient, professional, compassionate and caring manner. Directs the activities of paraprofessional as they provide care. Responsible for assuring effective communication with patients, support persons and members of the healthcare team. Unique populations include the underserved, refugees, homeless, Spanish speaking, and Mennonite to name a few. RESPONSIBILITIES: Clinical Responsibilities: • Conducts pre-visit planning (PVP). Works reports for gaps and delinquencies (patient outreach and scheduling for follow up). Completes health maintenance, registry reports (gap work) • Accurately obtains and documents patient vital signs, history, and screenings. Recognizes deviations from the norms, and promptly reports to a licensed staff member. • Understands principles of obtaining an accurate EKG and attaches monitor leads and operates equipment when directed.
