Develops a child health care plan in collaboration with the child’s medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF/CP&P. Links the child and caregiver with appropriate health resources and community services. Assures that the child is scheduled for and attends timely EPSDT visits and specialty appointments, reviews all reports, and ensures all necessary reports are forwarded to the medical home.
