The LPN Medication Reconciliation Nurse will perform medication reconciliation of orders with our contracted facilities’ Medication Administration Records (MARs) and Electronic Medical Records (EMR) to ensure accuracy and avoid medication errors. They will coordinate the process for any corrections and ensure follow-up was completed. Complete monthly medication reconciliation of facility MARs with InnovAge medication list.
