Implement and monitor patient care plans. Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient. Perform accurate and complete admission assessments and reassessments within a specified time frame. Assess patient for safety risks, abuse and neglect. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Assess and document patients' pain using appropriate pain scale and manage patients' pain then. Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Escalate change in patient condition to patient’s physician as appropriate. Scheduled Weekly Hours: 40 Work Shift: Days (United States of America) Hours: Monday - Thursday 10am - 9pm Job Summary: The Registered Nurse (RN) plans, organizes and directs hospice care. The RN supervises and coordinates care to patients with an array of terminal illnesses, diseases and compromised health states. The RN provides comprehensive skilled nursing services to qualified patients in their homes or hospice inpatient setting, so that the stated goals and objectives of the agency can be achieved. The nurse works with minimum daily supervision or support while performing interdependent practice in the field. The RN is responsible for supervising the hospice aide, administering medications, symptom assessment and management, education/teaching, and end-of-life care. Minimum Qualifications: Education: Graduate of an accredited school of nursing. Experience: One (1) year of professional nursing experience in an acute care, home care setting or hospice inpatient unit preferred. Hospice nursing experience preferred. Licensure/Certification: Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.
