Position Summary T his posting is for a Hematology RN Case Manager in the Children's MN Cancer & Blood Disorders Clinic. A hematology case manager is responsible for coordinating and overseeing the daily care of children with blood disorders to ensure their treatment plans run smoothly and safely. On a typical day, they review patient charts and treatment orders, coordinate appointments for transfusions, labs, and specialty visits, and monitor patients for complications or side effects. They serve as a central point of communication between providers, patients, and families, provide education on treatments and home care, arrange referrals and support services, and advocate for patient needs. Throughout the day, they document care, collaborate with the multidisciplinary team, and work to ensure no aspect of the patient’s medical or psychosocial care is overlooked. * 4 year degree BSN/BAN * Current MN RN license. * Current BLS Certification. * APHON Chemotherapy/biotherapy Certification obtained within 8 weeks of hire. * CPON preferred. * 3 years of current Pediatric Hematology Oncology experience preferred. Hennepin Health is seeking a Special Needs Basic Care (SNBC) Community Health Specialist to join the SNBC team to provide care navigation for members who are certified disabled focusing on the needs of members with mild-moderate complexity. This position will engage directly with members to offer care navigation and support including establishing primary care, addressing systemic barriers, and facilitating other supportive resources. This SNBC Community Health Specialist will work within an interdisciplinary team of nurses, other community health specialists, and a social worker to sensitively address the social determinants of health of SNBC members. This role will focus on a vulnerable population and will work to address their needs in a trauma-informed and member-centric way. Individuals filling this role must be flexible, innovative, and understanding of the challenges and unique health conditions experienced within the SNBC population. Services provided include home care and home care pharmacy, hospice care, palliative care, perinatal hospice program, and Children's Institute for Palliative Care. At PrairieCare, we are seeking dedicated Psychiatric/Mental Health RNs to provide exceptional care for our adolescent (ages 4-18) and young adult (ages 18-35) patients facing psychiatric and mental health challenges. Shift: AM/PM 32 hrs a week, every other weekend What You’ll Do: Conduct impactful assessments and monitor daily functioning to guide patient care. Use your critical thinking skills to deliver safe, high-quality nursing care. Collaborate with a dynamic multidisciplinary team to enhance patient outcomes. Create a nurturing therapeutic environment while minimizing restraint and seclusion. Engage patients through planning and facilitating exciting nursing groups. Administer medications accurately and empower patients and families with education. Provide compassionate, culturally sensitive care while maintaining confidentiality. Skillfully intervene in crisis situations using effective de-escalation techniques. Qualifications Education: Graduate of an accredited nursing school with a Nursing Degree; BSN preferred. Licensure: Current RN licensure with the Minnesota State Board of Nursing is a must. Experience: Ideally, you bring at least 1 year of psychiatric/mental health nursing experience within the past two years. Certifications: Professional certifications such as RN ANCC Certification in Mental Health Nursing, Pediatric Nursing, or Holistic Health are a fantastic bonus!
