Adheres to Nursing Policies and Procedures and Organizational Goals and Objectives. Essential Duties and Responsibilities: Clinical Practice 1. Provides age appropriate care for the population served. Gives consideration to growth & developmental phase of life, to age related factors such as thermal regulation, nutrition, medication administration, nutrition, safety and functional needs of the patient. 2. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs. 3. Demonstrates competency in assessment skills (Physical, spiritual, psychological, social) of the Hematology and Oncology Services patient. 4. Performs telephone triage for patient problems, utilizes decision trees to formulate plan of care. Consults with physician and CRNP as necessary to problem solve and direct urgent/emergent patient care. 5. Performs follow up telephone alls to evaluate Hematology and Oncology Services patient outcomes. 6. Demonstrates the ability to make correct clinical decisions and prioritize nursing actions. 7. Performs procedures according to policy and procedure. 8. Demonstrates competence in starting IV ports, performing therapeutic phlebotomy, intra- peritoneal chemotherapy, and intravenous chemotherapy administration. 9. Demonstrates competence with peripheral IV insertion. 10. Demonstrates competence with assessing and maintaining peripheral and central IV access. 11. Follows policy and procedure when using Cathflo to de-clot port access. 12. Adheres to the 5 rights of medication administration. Reports errors and/or adverse drug effects 100% of the time. Researches unfamiliar medications prior to administration. 13. Follows P&P for medication administration. 14. Adheres to aseptic technique as appropriate. 15. Uses resources and references for reviewing policies and procedures, medications, and clinical information. 16. Recognizes the need for and follows established protocols for requesting a physician’s order/intervention or obtaining medical attention for a patient in distress. Communication 1. Uses effective communication during hand-offs. 2. Collaborates with patient/family and other health care team members in planning care, teaching as documented on medical record. 3. Communicates special needs and/or precautions for patients to other Departments involved in the care of the patient. 4. Communicates patient care information to the interdisciplinary care team in a clear, accurate and professional manner. 5. Provides patient and family education in a manner that is understood and gives the patient and family handouts, brochures and resources that can be taken home for future reference. Documentation 1. Documents all therapies provided accurately. 2. Documents patient care that is relevant to patient’s diagnosis, problems and reflects patient’s status. Professional Role Development 1. Maintains equipment in working order. Uses equipment appropriately, follows Department policies for equipment and supply use and maintenance. 2. Performs assigned duties to maintain unit in a state of survey readiness and maintain quality initiatives. For example, quality control measures, equipment checks, medication preparation area cleanliness, etc. 3. Integrates principles of infection control & infection prevention into practice. 4. Uses personal protective equipment as appropriate and per policy for Chemotherapy Administration Standard Precautions and prevention of injury. (e.g. gloves face mask/shield, etc.). Management Responsibilities : 1. Ensures the patient care area is cared for, maintained, clean, and secure for patients, visitors, and staff 2. Assists the Director and Clinical Manager with compliance for policies, procedures, regulations, and standards of care. 3. Completes or delegates shift appropriate inventory and quality control checks: supply inventory, Waived test quality control (protein urine dipstick), billing information. 4. Integrates interdepartmental communication as evidenced by problem solving on the department level. 5. Assignments and coordination of care to support operations of the department. Education Training and Experience: Bachelors of Science in Nursing (BSN) is required. A minimum of 3 years recent nursing experience as an RN required. Must have strong IV therapy skills. Experience preferred in the specialty of oncology nursing. Oncology and Chemotherapy Administration experience strongly preferred. Excellent communication and patient education skills needed. Basic computer skills: keyboard, mouse use. Ability to complete data entry on clinical documentation screens. Ability to use email and internet. Ability to practice critical thinking and organizational skills . Ability to successfully complete on the job training for CCH computer programs. Ability to speak, read, write and understand English. Licensure/Certification: Current Pennsylvania RN License. CPR certification required. Chemotherapy certification strongly preferred; and required within first 3 months of employment. Full-Time Program Coordinator RN Location: 3400 Civic Ctr Blvd Schedule: M-F Day Penn Medicine’s Division of Hematology-Oncology is hiring a Global Health RN Coordinator. The RN Coordinator provides direct and indirect care to high acuity patients traveling to Penn Medicine from abroad. This position is responsible for the seamless integration of patients to have high-quality care at Penn Medicine followed by a comprehensive plan once they return to their home country. Penn Medicine's Division of Hematology/Oncology is one of the largest hematology/oncology groups in the country. We are recognized nationally and internationally for our excellence in patient care, research, and training. Our physicians, advanced practice providers, and nurses care for patients with all types of cancer and benign blood-related disorders, from the most common to the most complex, seeing more than 175,000 outpatient visits annually. The Global Health RN Program Coordinator provides clinical oversight and direct patient navigation of the specific patient population through the Department. Under the direction of the Department Clinical Leadership and Division Section Chief/Departmental Chair, the incumbent will have oversight of the development and implementation and day-to-day administrative functions of the program ensuring coordination throughout the patient continuum in collaboration with associated care teams. The scope of this work includes anticipating and facilitating multimodality dynamic scheduling for patients, triage services, development and reporting of key metrics, and coordination of care across multiple specialties and services. The RN Coordinator is responsible for swiftly reviewing cases with internal teams, converting opinions to cases thus steering new volume to Penn Medicine from around the globe in a highly competitive landscape. Referred cases are complex, e.g. co-morbidities, cultural challenges beyond English not as the first language, family joining in care decisions, and approvals from sponsors, which include government agencies, insurers or third parties, requiring coordinating complex transfers from great distances and coordinating delivery teams and time of arrival per various time zones. The RN Coordinator is responsible for coordination of care and services including but not limited to: complete assessment with limited medical records from patient, requesting studies per clinical protocols to ensure studies are completed prior to arrival, securing privacy forms from country and HIPAA, assessing level of complex referrals for patients within the UPHS system, working with faculty and staff to participate in care and assist with cultural awareness, as well as discharge planning, to include in country follow up services. Unlike any other position, creating a clinical care network upon discharge that meets Penn Medicine's standards of care is the quality metric essential to this role. Job Summary The Bilingual Certified Medical Assistant administers and performs triage and direct care to assigned patients under the direction and supervision of the RN/Nurse Manager.
