The Day Habilitation Program serves adults ages 22 and over with developmental disabilities to engage in therapeutic and recreational programming. The day program nurse provides daily nursing services including medication administration, g-tube feeding administration, first aid, and more. This role offers an opportunity to provide meaningful, person-centered care to individuals with intellectual and developmental disabilities, chronic health conditions, and complex medical needs in a supportive, community-based setting. The Adult Day Health RN is responsible for assessing, coordinating, and delivering nursing care to program participants during daytime services. This position plays a critical role in supporting individualized health plans, monitoring participant wellness, administering medications and treatments, and responding to changes in condition. Working collaboratively with an interdisciplinary team, the RN helps ensure safe program operations, regulatory compliance, and high-quality care that promotes independence, health, and overall quality of life. Responsibilities Clinical Care & Health Monitoring Conduct nursing assessments and ongoing health monitoring of participants Administer medications, treatments, and nursing interventions in accordance with physician orders and regulatory standards Identify and respond to changes in medical condition, communicating updates to providers and team members Maintain accurate and timely medical documentation, including MARs, assessments, and progress notes Care Coordination & Collaboration Collaborate with program staff, therapists, case managers, families, and healthcare providers to implement individualized care plans Participate in interdisciplinary meetings and care coordination efforts Support continuity of care and positive health outcomes for all participants Program Oversight & Compliance Provide nursing oversight for medication administration and health-related supports within the program Ensure compliance with all state, federal, and local Adult Day Health regulations Promote infection control practices and safe medication storage and handling Monitor health-related trends and participate in quality assurance and improvement initiatives Training & Support Assist in training staff on medication administration, health monitoring, infection control, and emergency response procedures Provide clinical guidance and support to program staff to strengthen care delivery Core Competencies Clinical expertise and sound nursing judgment Effective communication and interdisciplinary collaboration Strong documentation and compliance practices Commitment to person-centered care and participant advocacy Ability to manage multiple priorities in a dynamic environment Qualifications Active Registered Nurse (RN) license required Associate’s degree in Nursing required 1–3 years of nursing experience Valid Class C or Class D driver’s license
