The Center for Team Care (CTC) is a workforce wellness initiative within the VCU Health system to deliver high value, comprehensive, and concierge like primary care to VCU Health employees and dependents. The CTC is seeking experienced primary care APPs who can deliver high quality, high value care with a focus on population management and uncompromising patient experience. The ideal candidate will have experience working in a busy primary care practice with a focus on value based or concierge care. The ideal candidate will have experience delivering care in a holistic and lifestyle grounded approach. The candidate will serve as a PCP to a complex panel of patients, delivering high quality acute and episodic care, chronic disease management, and wellness care.
